Accelerated Transcranial Magnetic Stimulation (aTMS): Advancing Speed and Accessibility in Neuropsychiatric Care

Executive Summary

Accelerated transcranial magnetic stimulation (aTMS) refers to treatment paradigms in which multiple TMS sessions (≥2) are administered per day, thereby shortening the overall treatment course relative to conventional once-daily protocols.

This approach can be applied to established rTMS and iTBS therapies and is intended to address practical limitations of standard care, including extended treatment duration and associated barriers to access, adherence, and treatment completion.

Across randomized controlled trials, observational studies, and real-world data, aTMS demonstrates antidepressant efficacy broadly comparable to standard TMS. Reported response rates typically range from 40–60%, with remission rates between 25–40% in routine clinical settings.

While high-intensity protocols i.e., 10-20 treatments per day have reported higher remission rates and faster symptom improvement, these findings still require further confirmation in larger multi-arm randomized controlled trials.

The safety and tolerability profile of aTMS appears comparable to conventional once-daily TMS, with mild and transient adverse events and no clear increase in serious risks across studied protocols.
